On July 2, 2022, during the 25th National Forum on Interventional Cardiology (CCIF 2022), RainMed Medical successfully held a special meeting and a satellite symposium on coronary physiology. RainMed invited authoritative experts in the industry to make special speeches, share, comment and discuss the cases. During the forum, the research progress, clinical significance and application scenarios of caFFR and caIMR based on coronary angiography were also presented.

 

Jianping Li: caFFR and caIMR provide a convenient and rapid assessment of myocardial ischemia, thereby improving the clinical application rate of coronary functional analysis.

 

Professor Jianping Li gave a special report on the development of functional evaluation in the diagnosis and treatment of coronary disease. He pointed out that FFR is currently the gold standard for coronary functional evaluation, and the application of FFR to guide PCI can improve the prognosis.Coronary microcirculation dysfunction is also an important cause of myocardial ischemia, and IMR is recommended to evaluate coronary microcirculation function by guidelines. Compared with traditional guide wire evaluation techniques, both caFFR and caIMR can provide a convenient and rapid assessment of myocardial ischemia, thereby improving the clinical application rate of coronary functional analysis.The combination of caFFR and caIMR can evaluate myocardial ischemia conveniently and quickly, improve the application rate of coronary functional analysis and precisely guide the treatment of coronary heart disease.

 

 

Bei Shi: IMR is an important indicator to evaluate microvascular function, and RainMeds caIMR is a powerful substitute for guide wire-based IMR

 

Professor Bei Shi gave a special report on the understanding and progress of the index of coronary microcirculation resistance(IMR). She pointed out that coronary microvascular disease (CMVD) is common and the prognosis is poor. IMR is an important index to evaluate microvascular function. Compared with CFR, IMR can specifically reflect coronary microcirculation function, and its repeatability is higher than CFR. As to this aspect, RainMeds caIMR is safe, convenient, and efficient and could serve as a valid substitute for guide wire-based IMR, which requires no pressure guide wire, hyperemic agents, or SPSS injection.

 

 

Yuncheng Zou: RainMed caIMR provides an opportunity for the wide application of coronary microcirculation function evaluation

 

Prof. Yuncheng Zou introduced the latest clinical application progress of RainMeds caIMR. He pointed out that IMR is a suitable microcirculation evaluation technique for cathlab, and it has high repeatability and is not affected by blood pressure, heart rate, age, gender and other factors. RainMeds caIMR can calculate IMR directly based on two angiograms without the use of pressure guide wires, hyperemic agents, and SPSS injections. It provides an opportunity for the wide application of coronary microcirculation function evaluation.

 

Zheng Shen: Endovascular imaging is mainly used to guide PCI in complex lesions and it is not recommended for direct functional evaluation

 

Prof. Zheng Shen discussed the choice between FFR and endovascular imaging. He pointed out that FFR is a functional evaluation method, which is used to determine whether coronary stenosis causes myocardial ischemia and whether PCI is needed, and to evaluate the effect and prognosis of PCI. Endovascular imaging is mainly used for patients with complex lesions requiring PCI to guide and optimize the process of interventional therapy. Guidelines and expert consensus do not recommend direct use of endovascular imaging for functional assessment. For interventional treatment of complex lesions, the combination of FFR and endovascular imaging can better improve prognosis.

 

 

Xinjin Zhang: FFR-guided CABG can simplify surgical procedures without compromising prognosis

 

Prof. Xinjin Zhang shared the application of FFR in coronary artery bypass grafting (CABG). He pointed out that the role of FFR in PCI is indisputable, and its application in CABG is currently being investigated. The results showed that FFR decreased more significantly and had a higher occlusion rate after bypass compared with normal target vessels. The corresponding bridge vessels were more easily occluded after bypass. FFR-guided CABG can simplify surgical procedures without compromising prognosis. He is very optimistic about the use of RainMeds caFFR in guiding CABG.

 

 

Dong Huang: RainMeds caIMR provides an opportunity for large-scale functional diagnosis of coronary microcirculation

 

Prof. Dong Huang presented the results of the RainMed caIMR clinical trial. Flash III trial is a prospective, multicenter, blinded design registered clinical experiment to evaluate the efficacy and safety of caIMR. Result showed that the accuracy of online and offline measurement was as high as 93.81% and 94.69% respectively. Compared with the guide wire IMR, it has high sensitivity and speciality.

 

 

Yi Zhang: RainMed caFFR can directly calculate the FFR and the pressure difference of the whole vessel, which is especially suitable for precise guidance of interventional treatment of tandem and diffuse lesions

 

Professor Yi Zhang gave a special report on the treatment strategy of tandem and diffuse lesions guided by RainMeds caFFR. He pointed out that it is necessary to determine whether those lesions require interventional treatment and how to reduce the number of stents. Compared with other assessment methods, FFR can accurately distinguish whether tandem or diffuse lesions are causing ischemia and accurately locate the "culprit" segment. During the treatment, RainMeds caFFR does not need to withdraw the guidewire. The whole caFFR curve can be calculated directly according to the angiograms, and the pressure difference table can be given, which is especially suitable for precise guidance of interventional treatment of tandem and diffuse lesions.

 

 

The conference presented the clinical advantages of RainMeds caFFR and caIMR, which are non-interventional coronary functional indicators, and provided an opportunity to improve the clinical application rate of coronary functional evaluation and promote precise coronary diagnosis and treatment.
