On October 6th, 2021, RainMed Satellite Conference was successfully held in Rizhao, Shandong province during the 6th Shanghai and Shandong Expert Forum on Cardiovascular Diseases. Renowned experts in cardiovascular field from Shanghai and Shandong discussed the significance and prospect of functional diagnosis and clinical application, and pointed out that caFFR testing after PCI can improve the prognosis effect of patients and save medical resources.

During the meeting, Professor Xiaoyan Wu pointed out that FFR has been widely used as a diagnostic tool for preoperative myocardial ischemia for a long time. In fact,  FFR also plays an important role in postoperative detection.

A large number of studies have shown that postoperative FFR value is significantly correlated with the outcome: the higher the postoperative FFR value and the FFR difference between the two ends of the stent, the lower the incidence of adverse events and the better the prognosis. When FFR detection was performed on patients undergoing stent surgery, researchers found that FFR results were not ideal in a large number of patients after surgery. FFR values were low due to the type of lesions and stent implantation scheme, and good prognosis could still be achieved after remedy by drugs, balloon dilation, stent implantation and other means. However, the result drift caused by the limitation of technology has misled the surgeon.

As an innovative non-interventional treatment, caFFR does not require vasodillators and has 95.7% test consistency compared with guidewire FFR. It can effectively replace guidewire FFR, improve the prognosis of patients, and save medical resources.

At the same time, experts pointed out that for patients with moderate stenosis, 1/3 of the patients with ischemia will be ignored only by angiograms, which is easy to miss the opportunity of PCI treatment. caFFR is evaluated from a functional perspective, and the whole vessel FFR value can be obtained by a single measurement, which can help guide the diagnosis and treatment of multi-vessel, diffuse and bifurcation lesions.

Professor Dawei Yu demonstrated the accuracy and convenience of caFFR through case sharing. He believes caFFR can optimize clinical diagnosis and treatment strategies and promote the development of coronary physiology. Optimized caFFR has a wide range of applications, providing a good opportunity for the clinical promotion of functional evaluation and the development of precision medicine.
