On July 3rd, 2022, during the 17th Ice City Conference of Cardiology (ICC 2022), RainMed Medical successfully held the Physiological Function Forum. Renowned cardiovascular experts from all over the country had a heated discussion on research progress, clinical significance and application scenarios of caFFR and caIMR.

 

On the forum, Prof. Bo Yu from The Second Affiliated Hospital of Harbin Medical University served as the chairman. Prof. Dalin Jia from the first hospital of China Medical University, Prof. Bin Liu from The Second Hospital of Jilin University, Prof. Wenyue Pang from Shengjing Hospital of China Medical University, Prof. Wei Yang from The Fourth Affiliated Hospital of Harbin Medical University served as the host. Prof. Zhiqi Sun from Daqing Oilfield General Hospital, Prof. Jingbo Hou from The Second Affiliated Hospital of Harbin Medical University, Prof. Zhaoqing Sun from Shengjing Hospital of China Medical University, Prof. Bo Li from The Second Hospital of Jilin University, Prof. Xingli Liu from Shengjing Hospital of China Medical University and Prof. Yunlong Huo from Shanghai Jiao Tong University served as the speakers.

 

 

Jingbo Hou: caFFR and caIMR provide a convenient and rapid assessment of myocardial ischemia, thereby improving the clinical application rate of coronary functional analysis.

 

Professor Jingbo Hou gave a special report on the development of functional evaluation in the diagnosis and treatment of coronary disease. He pointed out that FFR is currently the gold standard for coronary functional evaluation, and the application of FFR-guided PCI has changed the treatment strategy of one-third of patients and improved the prognosis. Coronary microcirculation dysfunction is also an important cause of myocardial ischemia, and IMR is recommended to evaluate coronary microcirculation function by guidelines. The combination of caFFR and caIMR can evaluate myocardial ischemia conveniently and quickly, improve the application rate of coronary functional analysis and precisely guide the treatment of coronary heart disease.

 

 

Zhiqi Sun: FFR measurement after PCI has significant prognostic value. Compared with guidewire-based FFR, RainMeds caFFR can significantly simplify the postoperative evaluation process

 

Professor Zhiqi Sun gave a keynote speech on the postoperative value of FFR and caFFR's advantages. He pointed out that FFR measurement after PCI has been proved to have significant prognostic value, and it is also recommended by experts to evaluate the effect of stent implantation. caFFR is an advanced FFR evaluation method that provides intuitive and detailed inspection reports without retraction or verification. It can quickly locate the problem segments, provide greater convenience and safety than guidewire-based FFR, and significantly simplify the postoperative evaluation process.

 

 

Yunlong Huo: RainMed Medical will launch a robotic system for vascular interventional surgery that covers the whole process of diagnosis and treatment, providing an overall solution for interventional surgery

 

Professor Yunlong Huo shared the project progress of RainMeds vascular interventional surgery robot. The procedure for vascular intervention is complicated, Huo said, current robots can only place stents for doctors. Therefore, RainMed Medical will launch an automated vascular interventional surgery robot system that covers the whole process of diagnosis and treatment, including coronary, peripheral, nerve and other major interventional procedures, thereby assisting doctors to carry out efficient, accurate and standardized operation, so as to provide the overall solution of interventional surgery. The digital functional diagnosis module including caFFR system and caIMR system is currently available. It can provide complete functional evaluation of the vessels from epicardial coronary artery to coronary microcirculation, significantly improve the clinical application rate of coronary functional indicators, and promote the accurate diagnosis and treatment of coronary heart disease.

 

 

Zhaoqing Sun: RainMed caFFR can directly calculate the FFR and the pressure difference of the whole vessel, which is especially suitable for precise guidance of interventional treatment of tandem and diffuse lesions

 

Prof. Zhaoqing Sun gave a special report on the treatment strategy of tandem and diffuse lesions guided by RainMeds caFFR. He pointed out that it is necessary to determine whether those lesions require interventional treatment and how to reduce the number of stents. Compared with other assessment methods, FFR can accurately distinguish whether tandem or diffuse lesions are causing ischemia and accurately locate the "culprit" segment. During the treatment, RainMeds caFFR does not need to withdraw the guidewire. The whole caFFR curve can be calculated directly according to the angiograms, and the pressure difference table can be given, which is especially suitable for precise guidance of interventional treatment of tandem and diffuse lesions.

 

 

Bo Li: FFR-guided complete revascularization in STEMI patients significantly reduces composite endpoint events, so we recommend RainMed's caFFR for nonculprit vascular intervention treatment

 

Prof. Bo Li shared the research progress of FFR-guided non-culprit vascular treatment in STEMI patients. In recent years, with the development of medicine and pharmacy, lots of studies have supported the revascularization of non-culprit vessels in STEMI patients, Li said. FFR-guided revascularization did not increase any surgical complications, but significantly reduced revascularization rates, and thus significantly reduced composite endpoint events. The most recent U.S. guidelines recommend staged PCI for functional non-infarcted artery stenosis to reduce the risk of death or MI (1,A). Compared with traditional FFR measurement methods, RainMeds caFFR is safe, efficient, simple and accurate, which can greatly improve the clinical application rate of FFR, said Professor Bo Li.

 

 

Xingli Liu: endovascular imaging is mainly used to guide PCI in complex lesions and it is not recommended for direct functional evaluation

 

Prof. Xingli Liu discussed the choice between FFR and endovascular imaging. He pointed out that FFR is a functional evaluation method, which is used to determine whether coronary stenosis causes myocardial ischemia and whether PCI is needed, and to evaluate the effect and prognosis of PCI. Endovascular imaging is mainly used for patients with complex lesions requiring PCI to guide and optimize the process of interventional therapy. Guidelines and expert consensus do not recommend direct use of endovascular imaging for functional assessment. For interventional treatment of complex lesions, the combination of FFR and endovascular imaging can better improve prognosis.
